A website has been established to allow providers to view and
download samples of the four CERT Documentation Request Letters along
with the Request for Additional Documentation Letter (formerly
identified as a Tech Stop letter).
Also located at this website, providers are able to view and update
their medical record contact information including: provider name, point
of contact for medical records, address, email address, phone and fax
numbers to ensure medical record requests from the CERT Documentation
Contractor (CDC) are sent to the appropriate individual. The website
address is

Medicare providers can confirm that the CDC has the correct contact
information by clicking on the Provider Address Directory link at the
website location. On the Provider Address Directory page, the provider
will enter their Medicare Provider Number to initiate a search for their
contact information. If changes are needed to the listed information,
providers can contact the CDC at 301-957-2380 or by clicking on the link
to send the CDC an email with the updated contact information. If the
provider is unable to locate their contact information in the CDC
database, the provider is encouraged to submit the information to the
CDC.
